Now Accepting Eligibility Documents for the 2024-2025 School Year
Athletic Sign-Up (link to form)
- For 2025-2026 seasons physicals must be dated no earlier than 4/1/24
- Fill out first 3 pages before appointment
- A medical professional will fill out the last page for clearance to participate in sports
- Must be completed annually
- There are several pages that require a signature. Be sure to sign all agreements.
- No longer available to be completed online; paper copies can be printed or are available in the HS office
